Description: This book of common dermatologic differential diagnoses includes many clinical photos as well as algorithms.
Purpose: According to the authors, the aim is to help primary care physicians reach correct dermatologic diagnoses during patient encounters. These worthy objectives are, for the most part, met.
Audience: The book is intended for medical students, as well as residents and practitioners who are not dermatologists. It would be most helpful for those in internal medicine or family practice.
Features: Comprehensive instructions on how to precisely describe skin lesions begin the book, setting the foundation for all subsequent chapters. The main part of the book covers diagnoses grouped according to the part of the body affected, as well as a brief section on the treatment of each of the diagnoses. A separate chapter covers dermatologic treatment and includes topical cortisone potencies and tables on wound care. Lastly, the book provides a list of drugs and associated skin reactions. What is best about the book is that it is easy to use at the bedside. When faced with an unknown rash, the physician can narrow the possibilities by looking in the appropriate chapter based on body part involved, and then hone it down further by the prompts given in the algorithms. The accompanying photographs and text make the process easier still. The disease descriptions are concise and pertinent. The algorithms and tables are clear and easy to use. The only shortcoming is that a few of the descriptive terms in the algorithms are a bit confusing, such as the classification of discoid lupus in the "normal surface" category while showing a scaly lesion of it. These oversights are infrequent and quite minor, however, and in no way detract from the usefulness of the book. One of my quests in life is to produce, or preferably find, a really great differential diagnosis book, and it's a shame that the authors did not write a similar book aimed at dermatologists. Unfortunately, this book is really not broad enough in its differential diagnoses to be of use to the specialist.
Assessment: This is a valuable tool and a must for any nondermatologist who is bound to encounter patients with skin disease. Its strength lies in the organization of a bewildering array of skin growths and rashes into manageable logical categories and easily followed algorithms. It is both concise and complete. Along with a more traditional review text of dermatology, this should be the second of two books on dermatology every internist or family practitioner owns.
